Emerge Connect
Emerge Connect is a community-based recovery program that empowers individuals through peer support, advocacy, and access to personal development and essential recovery resources, such as in having a peer advocate, engaging in transformative group session offerings (virtual and in-person), community engagement, and case management. The only requirement to be a member of Emerge Connect is a commitment to recovery through community.
An extension program, Emerge Connect Wireless*, is offered to Emerge Connect members (Peers) who request an active cell phone with data plan and/or a laptop, which has accountability requirements designed to encourage recovery, nurture connection, and provide supportive guidance.
Applying to Emerge Connect Wireless*
Once a member of Emerge Connect, Peers who request a cell phone and/or laptop will be asked to participate in a 3+ month peer advocate accountability program.
Peers will be offered a laptop and/or an active cell phone with data access, assigned a peer advocate, and provided resource management.
Peers will be asked to follow a set of requirements designed to encourage recovery, nurture connection, and provide supportive guidance.
What to expect
Within 48 hours from the time applications are received we will reach out to the recovery professional sponsoring you (case manager, peer advocate, social worker, or therapist).
Upon approval, a laptop and/or phone will be delivered to the sponsoring individual.
Peer Commitment
Scheduled peer advocate sessions once per week
Scheduled therapy session once per month
Peer group session participation twice per month
Continued enrollment in local medication management services through a MAT provider or psychiatrist.
Application Requirements
A letter stating how phone access will support your recovery efforts.
Participation from a related case manager, peer coach, social worker, or therapist that will sponsor a peer by providing a positive recommendation.
Peers need to demonstrate a desire to follow through with a sustainable recovery plan, having done so as well in their current treatment. Peers need to also demonstrate a need, where they otherwise wouldn't be able have access to a reliable phone and service.

Peer needs to submit application.
Step 2 Sponsor Input
Upon Peer Application submission, their Sponsor will receive a link to a Sponsor Input form.
Step 3 Enrollment Instructions
Upon review, Sponsor will receive follow-up questions or an approval. The approval email will come with enrollment instructions.
Please note time sensitivity:
Steps 1 & 2 must be completed within 7 days of a patients discharge date, if at a residential facility.
Step 3, enrollment process, must be completed 3 days before discharge date, if at a residential facility.
Phone will be delivered within 3 days from enrollment completion.
